Checklist for your in-patient admission

This is what you should bring with you to the hospital:

Health insurance card
Referral slip (from a general practitioner or specialist)
If applicable, proof of supplementary insurance
Doctor's letters and current examination results (e.g. laboratory results, ECG, X-rays)
If applicable, further documents, e.g. vaccination book, X-ray passport, blood sugar diary, diabetic passport
List of medicines currently being taken
